Letter to ENYRNA groups regarding reopening meetings

Certificates of Insurance for a new group or request by a facility provider.

All groups that are a member of Suffolk Area Service NA must obtain a Certificate of Insurance when starting a new meeting or when the provider of the group’s existing meeting place requests a Certificate of Insurance.

To acquire an insurance certificate, please follow these instructions:

  1. Fill out the Group Registration form.
  2. Fill out the form completely and accurately by typing in all the information.
  3. Make sure there is a valid email address filled in on the form, as well as a phone number of a home group member.
  4. In the meeting time field, add one hour before the start of meeting and one hour at end of meeting for cleanup.
  5. Save the document to your computer. Note: there is no need to print this document.
  6. Email completed form to Insurance Liaison.
  7. Once it is processed, the Insurance Liaison will then email the insurance certificate to the contact email address entered on the form.
  8. You can expect to receive the insurance certificate in 7-10 days.

Group Events:      

If a group is having an event during its normal meeting date and time, the regional insurance coverage for member’s damage to a facility will be in force and a separate Certificate of Insurance is not needed unless specifically requested by the facility or location.

If there will be swimming or sports at that event, a Special Event’s Questionnaire needs to be filled out and the notes below will apply. It is the group’s responsibility to inquire with the facility regarding the required insurance coverage.

If the group event is at the group’s meeting location but, at a different date and or time, the regional insurance coverage for member’s damage to a facility will be in force and a separate Certificate of Insurance is not needed unless specifically requested by the facility or location.

If there will be swimming or sports at the event, a Special Event’s Questionnaire needs to be filled out and the notes below will apply. It is the group’s responsibility to inquire with the facility regarding the required insurance coverage.

Certificates of Insurance for all subcommittee, area, convention fundraising and the convention itself, events.

When a subcommittee, area, convention fundraising and the convention itself needs insurance coverage for a facility and / or an event, the event representative needs to request a Certificate for Insurance for the specific date and time for the facility and / or event. A Special Event Questionnaire needs to be filled out and the notes below will apply. It is the subcommittee, area, convention fundraising and the convention itself responsibility to inquire with the facility regarding the required insurance coverage.

The Special Events Questionnaire

There will NEVER be a submission for a request to have the serving of alcohol covered.

As indicated on the form, if your event is requesting swimming or having any type of sporting event, you must provide the proper information requested on the questionnaire.

There may be an additional charge for the insurance rider for swimming and / or sports from the insurance carrier should the group desire insurance coverage from the regional insurance carrier.

  • The charge for the additional insurance rider is to be paid upfront by a group for its additional coverage once the group accepts the charge for the coverage to be provided under the additional insurance rider. Once the payment is made, the insurance liaison will obtain the Certificate of Insurance from the insurance carrier.
  • If a subcommittee, area, convention fundraising and the convention itself accepts the charge for the coverage to be provided under the additional insurance rider, that entity will arrange payment with the area treasurer for the insurance provided. Upon approval, the insurance liaison will obtain the Certificate of Insurance from the insurance carrier.

  2. Fill out the form completely and accurately by typing in all the information.
  3. Make sure there is a valid email address filled in on the form, as well as a phone number of a committee member.
  4. Make sure the facility contact information is filled in on the form.
  5. Add one hour before the start of the event and one hour at end of the event for cleanup.
  6. Save the document to your computer. Note: there is no need to print this document.
  7. Email completed form to Insurance Liaison.
  8. The insurance liaison will make their best effort to provide feedback to the requesting entity regarding their Special Events Questionnaire within seven to fourteen days after the form has been confirmed as received by the insurance liaison.

Suffolk Area, as per its policy, will not promote any event until it is confirmed that the sponsor of the event has proof of insurance that conforms to the event and its activities. The Insurance Liaison will offer assistance so the event will have proper insurance coverage and any promotional material will reflect conformity to enable promotion by Suffolk Area.
